Chapter ACH Bank Transfer Request

Please fill out this form to request that any money owed to your chapter be directly deposited into your chapter's bank account, such as a chapter rebate. This will replace the need to send checks and prevent the possibility of getting your money lost in transit.

Once this is set up, you will not need to put in another request unless your information changes. A KDP representative will reach out if anything on this form needs to be verified.
